Preliminary Information Form

The Preliminary Information Form (PIF) is designed to enable you to provide sufficient information for the State Historic Preservation Office (SHPO) staff to evaluate a property in terms of the criteria for the National Register of Historic Places. This form, when completed, will allow the State Historic Preservation Office to make a recommendation on whether or not it is advisable for you to undertake the time and expense involved in preparing an actual nomination.
